Apixaban is an anticoagulant indicated to reduce the risk of stroke and systemic clots, to reduce the risk of deep vein thrombosis which can lead to blood clots in the lungs following hip and knee replacement, and to treat and reduce the risk of deep vein thrombosis and blood clots in the lungs after initial therapy. Apixaban is a selective inhibitor of FXa, a serine trypsin-like protease. It does not require antithrombin III for antithrombotic activity. Apixaban inhibits free and clot-bound FXa, and prothrombinase activity. Apixaban has no direct effect on platelet aggregation, but indirectly inhibits platelet aggregation induced by thrombin. By inhibiting FXa, apixaban decreases thrombin generation and thrombus development which helps blood clots to form.
