Benzatropine (benztropine) is a centrally active muscarinic antagonist indicated as an adjunct in the treatment of parkinsonism and in the treatment of extrapyramidal symptoms. Benzatropine is a potent inhibitor of presynaptic carrier-mediated dopamine transport. As a known analog of atropine, it has a high affinity for muscarinic receptors in the human brain. Once bound, benzatropine blocks the activity of muscarinic receptors, mainly in the striatum. The antagonism of acetylcholine corrects the imbalance between dopamine and acetylcholine in Parkinson’s patients. Benzatropine may also block dopamine reuptake, thereby increasing dopaminergic activity. In animal studies, the anticholinergic activity of benzatropine was about equal to that of atropine.
