Please note, this OEL/ADE monograph also applies to carbachol glutamate and carbachol carbamate. Carbachol is a synthetic choline ester and a positively charged quaternary ammonium compound. Carbachol is a parasympathomimetic that mimics the effect of acetylcholine on both the muscarinic and nicotinic receptors. Carbachol administered intraocularly is indicated to induce miosis and to reduce the intensity of the intraocular pressure elevation in the first 24 hours after cataract surgery. The administration of topical ocular carbachol is also indicated in the treatment of glaucoma. Carbachol has also been used orally in the treatment of postoperative urinary retention and by inhalation for the induction of bronchoconstriction in asthma diagnosis.
