Difamilast is indicated for adult atopic dermatitis. It is under investigation in a clinical trial (NCT01702181) to treat adults with atopic dermatitis in the United States. Difamilast, a phosphodiesterase 4 (PDE4) inhibitor, selectively inhibits PDE4 subtypes (PDE4A, PDE4B, PDE4C, and PDE4D), with a particularly strong effect on PDE4B. Phosphodiesterase 4 (PDE4), found in various immune cells, specifically degrades cyclic adenosine 3',5'-monophosphate (cAMP). By increasing intracellular cAMP concentration, this drug controls the production of cytokines and chemokines, thus suppressing skin inflammation. It is anticipated to be effective in treating atopic dermatitis, a condition characterized by recurring episodes of itchy eczema, as patients with atopic dermatitis often exhibit increased PDE4-like activity and decreased intracellular cAMP concentration in peripheral blood leukocytes.
