Elafibranor is a multimodal and pluripotent medication under investigation for treatment of non-alcoholic fatty liver disease (NAFLD) including non-alcoholic steatohepatitis (NASH). Elafibranor is an agonist of the peroxisome proliferator-activated receptors (PPARs) α and δ. PPARs are nuclear receptors that play key roles in cellular processes regulating metabolic homeostasis, immune-inflammation, and differentiation. PPAR-α is most prominently expressed in the liver and controls the lipid flux in the liver by modulating fatty acid transport and improving plasma lipids. PPAR-δ (also called PPAR-β) regulates metabolism in liver and peripheral tissues. PPAR α and δ agonists enhance fatty acid transport and oxidation. Elafibranor enhances the activity of PPARs, which has a positive effect on obesity, insulin-resistance and diabetes, atherosclerosis, inflammation, and plasma lipids (increasing HDL, lowering triglycerides and LDL).
