Ephedrine is a mixed-acting oral sympathomimetic indicated for the treatment of asthma, drug induced low blood pressure, and the common cold. Ephedrine’s mechanism of action stems from stimulation of both alpha-adrenergic and betaadrenergic receptors. It exerts a direct effect on adrenergic receptors and an indirect action due to the release of norepinephrine. Ephedrine stimulates the central nervous system and increases blood pressure by vasoconstriction and increasing cardiac output. Stimulation of beta-adrenergic receptors in the lungs results in bronchodilation. Urinary retention occurs because of constriction of the sphincter muscle and relaxation of the urinary bladder wall. Pharmacologic actions of ephedrine include elevation of blood pressure, increased pulse rate, vasoconstriction and cardiac stimulation to increase output and force of contraction.
