Leucovorin calcium (also known as calcium folinate) is an active metabolite of folic acid (also called folinic acid and citrovorum factor), which does not require metabolism by dihydrofolate reductase, the molecular target of folate antagonist-type chemotherapeutic drugs. Leucovorin is an intermediate product in the metabolism of folic acid and occurs naturally in the body. It is indicated for the treatment of osteosarcoma (after high dose methotrexate therapy). It is used to diminish the toxicity and counteract the effects of impaired methotrexate elimination and of inadvertent overdosages of folic acid antagonists, and to treat megaloblastic anemias due to folic acid deficiency. Leucovorin calcium is also used in combination with 5-fluorouracil (5-FU) to prolong survival in the palliative treatment of patients with advanced colorectal cancer.
