Milrinone inhibits erythrocyte phosphodiesterase, resulting in an increase in erythrocyte cAMP activity. The erythrocyte membrane subsequently becomes more resistant to deformity. Milrinone also decreases blood viscosity by reducing plasma fibrinogen concentrations and increasing fibrinolytic activity. The inhibition of cAMP phosphodiesterase isozyme in cardiac and vascular muscle increases ionized calcium and contractile force.
