Obeticholic acid is a farnesoid X receptor (FXR) agonist used alone or in combination with ursodiol to treat liver disease caused by bile duct destruction (primary biliary cholangitis, PBC) and bile accumulation. Obeticholic acid is an agonist of the FXR a key regulator of bile acid, inflammatory, fibrotic, and metabolic pathways. Activation of FXR decreases concentrations of bile acids in liver cells by suppressing their synthesis and increasing their removal; thus, reducing exposure of the liver to bile acids.
