Stavudine is used to treat human immunodeficiency virus (HIV). Stavudine, a nucleoside analog of thymidine, is rapidly phosphorylated by cellular kinases to its active moiety, stavudine triphosphate. Stavudine triphosphate inhibits HIV replication by competing with the natural substrate, thymidine triphosphate, and by inhibiting viral DNA synthesis by acting as a terminator of chain elongation. In addition, stavudine triphosphate inhibits cellular DNA polymerases beta and gamma, and markedly reduces the synthesis of mitochondrial DNA.
